SIGN THE WAIVER

To access the treatment please download, fill and sign the waiver of liability and consent form before placing any order. We will not process orders before receiving the signed document.

EMAIL YOUR INFO

Please send the signed waiver together with the following info to west@cure4paws.com or east@cure4paws.com:

  • Referral (admin/other)

  • Name of the person placing the order (if different from the parent who signed the waiver)

  • Add a screenshot of your treatment chat with the invitation from your admin to our website

PLACE YOUR ORDER

Once we receive your signed waiver and the requested information we will send you the code to access to the ordering page. If you are experiencing issues please contact us.

HARMONY FIP RESEARCH - GS RESISTANCE PROGRAM

If your cat does not respond to GS-441524 treatment or if symptoms do not resolve at high dosages (e.g. 20mg/kg) and you have tried both the injectable and oral formulations of reliable sources of GS-441524 you can qualify for our GS resistance trials.
